Patient Confidentiality

We take patient confidentiality seriously and all personal information is treated with strict confidence. We ask you to fill in a medical history form which we will check on each visit. Please make a note of your prescribed medications given from your GP this Will assist us with best diagnosis and treatment.


Patient Responsibilities

• Follow your dentist's advice to prevent tooth decay and gum disease.

• Keep your appointments and regular checkups.

• Provide proof of exemption entitlement, if applicable, for the cost of NHS treatment.

• Pay all charges after each visit. For some treatment we may ask you for a deposit at the start of treatment.

• If you need to cancel an appointment, please allow us at least 24 hours' notice. If an appointment is not cancelled but also not attended, we may be unable to offer future treatment to you.

Sharing information

The information held about you will not be shared for any reason, unless:

• You ask us to do so

• We ask and you give us specific permission

• We are required by law, for example prescribers must be notified where a patient presents a repeat prescription arid the pharmacist becomes aware of clinicaily significant issues arising in connecticion with that prescription,

• We are permitted ny law, for example where public interest overrides the need to keep the information confidential.


Anyone who receives information from us also has a legal duty to keep this information confidential, subject to recognised exceptions of the types listed above.

We welcome comments, suggestions and complaints so that we can continually improve our service to you: Please contact Miss A. Jago in person or by phone, letter or email if you have a comment, suggestion or complaint.


We take complaints very seriously and have an effective procedure to resolve any problems in the shortest possible time. You can read more about our procedure in our complaints policy (available on request). We always want to have satisfied patients.
